This form is required unless prior approval is given by Paragon Equine.

 

 

Payment for services rendered is expected at the time of service or once the invoice has been finalized. Invoices will be written within 24 hours of appointment. Unless prior arrangements are made, we intend to charge the credit card and email the invoice.
